Service 03

Family Guidance & Education

Preparing families for every step of the journey.

At Aceso Hospice, our interdisciplinary team brings diverse clinical backgrounds and specialty expertise to the care of our patients and families. Many of our nurses and healthcare professionals have extensive experience in areas such as trauma care, emergency medicine, critical care, oncology, geriatrics, and palliative care.

This specialized experience enhances hospice care by providing advanced symptom management, rapid assessment skills, and a deep understanding of complex medical conditions. For example, clinicians with trauma and emergency care backgrounds are highly skilled at recognizing subtle changes in a patient's condition, managing pain and distressing symptoms, and responding calmly during challenging situations. Their expertise helps ensure patients remain comfortable, safe, and dignified throughout their end-of-life journey.

By combining specialty clinical knowledge with the compassionate philosophy of hospice, our team delivers holistic care that addresses not only physical needs, but also emotional, spiritual, and psychosocial well-being. This multidisciplinary approach allows us to create individualized care plans that honor each patient's goals, values, and wishes while providing unwavering support to their loved ones.

At Aceso Hospice, our diverse expertise strengthens our commitment to delivering exceptional comfort, dignity, and compassionate care when it matters most.

  • Caregiver training and education
  • What to expect as illness progresses
  • Guidance on advance directives and end-of-life wishes
  • Help navigating insurance and Medicare benefits
  • Coordination with family members across distances

Service 04

24/7 On-Call Support

We are always available — day or night.

Crises don't follow business hours. Our on-call clinical team is available around the clock to answer questions, provide guidance, and dispatch a nurse when needed. You will never face a frightening moment alone.

Free Hospice Assessments

At Aceso Hospice, our Triage Coordinator provides complimentary hospice assessments to help patients and families better understand their care options. These assessments evaluate the patient's medical condition, symptom burden, functional status, and overall goals of care to determine whether hospice services may be appropriate.

Working closely with physicians, discharge planners, and family members, our Triage Coordinator conducts timely, compassionate evaluations while answering questions and providing education about the hospice benefit. With clinical expertise in complex care management and support from our multidisciplinary team, we help ensure a smooth transition to comfort-focused care when the time is right.

  • Nurse available by phone 24 hours a day, 7 days a week
  • After-hours nurse visits when clinically necessary
  • Crisis intervention and continuous care during acute episodes
  • Inpatient hospice care when symptoms cannot be managed at home
  • Respite care to give family caregivers a rest

Coverage & Cost

Most services are covered at no cost to you

The Medicare Hospice Benefit covers virtually all hospice services — including nursing visits, medications, medical equipment, and counseling — with no deductibles or copays for most patients.

Medi-Cal and most private insurance plans also cover hospice care. Our team will verify your benefits and walk you through coverage before care begins — so there are no surprises.
